Typically users will call a number rather than fill out a form if your offering is a service as opposed to a product. Having that phone number in plain sight at the top right of the page (in the header) will help increase conversions. Also, you may want to think about using different phone numbers for different PPC campaigns if your conversion is strictly phone numbers.
